**Opportunity Highlights:**
- Full-time; 36 or more hours per week
- Day shift; 8am-5pm
- Seeking a skilled and compassionate Nurse Practitioner with a passion for Congestive Heart Failure care
- The NP or PA will play an important role in achieving program goals of reducing hospital readmissions, improve patient quality of life, support expansion of new treatment options for Cardio patients, improve access to care.
- Enjoy a mix of clinical and administrative responsibilities.
- Assess and coordinate the educational and psychosocial needs of the patient and family.
- Collaborate with CHF Program Director and other specialties to deliver individualized and timely care to a complex patient population.
- Practice independently as part of a high-functioning, progressive, multidisciplinary team.
- Practice in a mix of both inpatient and outpatient settings.
- Work with the team to share information and provide education to continually develop the team and to engage the team in quality patient care.

**Requirements:**
- NP must be a graduate of a nurse practitioner education program and have national certification.
- 3+ years of advanced practice experience in Cardiovascular Heart Failure required.
- Inpatient and outpatient Cardiology experience a plus.
- Completion of a Cardiology fellowship is desirable
- CHFN Certification is a plus.
- Must be comfortable independently managing assigned patients in a busy practice environment with minimal oversight from the supervising physician.
- Clean background NC licensure required.
