Job description

The Wound Care Specialist is responsible for assessing, managing, and treating patients with acute and chronic wounds across various care settings, including clinics, skilled nursing facilities, and home health environments.

Key responsibilities include performing wound assessments, conducting debridement procedures, and developing individualized treatment plans in collaboration with interdisciplinary care teams. The Wound Care Specialist should approach each patient encounter with clinical expertise, compassion, and professionalism, ensuring evidence-based, timely, and effective care that supports improved health outcomes and patient well-being.

Responsibilities

  • Perform comprehensive physical examinations and wound evaluations for patients with acute and chronic conditions.

  • Assess wound characteristics, review comorbidities, and coordinate treatment plans with managing providers.

  • Perform wound debridement and dressing changes according to clinical necessity and best practice standards.

  • Develop and implement individualized care plans in collaboration with medical and interdisciplinary teams.

  • Accurately document all assessments, procedures, and communications in the EMR system.

  • Complete all clinical documentation within 24 hours of each patient encounter.

  • Provide education to patients and caregivers on wound care, preventive strategies, and chronic condition management.

  • Communicate effectively with primary care providers, specialists, and care teams regarding patient status or changes.

  • Collaborate with medical assistants, clinical support staff, and leadership to maintain efficient workflow.

  • Participate in clinical meetings, training sessions, and quality improvement initiatives.

  • Advocate for patient needs and ensure access to appropriate medical resources and follow-up care.

  • Deliver compassionate, patient-focused education tailored to individual needs.

  • Maintain compliance with all clinical, safety, and regulatory standards.

  • Support organizational initiatives aimed at improving patient outcomes and operational performance.

  • Maintain professional licensure and engage in ongoing continuing education.

  • Stay current with CMS documentation and treatment guidelines.

Qualifications

Required:

  • Current, unrestricted Nurse Practitioner (Wound Care Specialist) or Physician Assistant license

  • Clinical experience in wound care or a related specialty.

  • Demonstrated proficiency in wound assessment, debridement, and care-plan development.

Preferred:

  • Wound care certification (WCC, CWCN, or equivalent).

  • Experience in skilled nursing facilities, home health, or outpatient wound care settings.

  • Familiarity with EMR systems and mobile charting.

  • Strong clinical judgment and ability to work independently.

  • Excellent verbal, written, and interpersonal communication skills.

  • Compassionate, patient-centered approach to care.

  • Ability to manage mobile schedules and adapt to changing priorities.

  • Knowledge of regenerative medicine, wound-healing technologies, and advanced modalities.

  • Ability to adopt emerging technologies and tools to enhance care delivery.

Work Environment

This is a field-based role requiring travel between clinical and community care sites. Frequent use of mobile technology and EMR systems is required. The position may involve exposure to patients with infectious or complex conditions; appropriate PPE will be provided.
The role regularly requires standing, walking, and traveling between locations. Some duties may involve bending, lifting, or moving medical equipment and supplies up to 25 pounds. Reasonable accommodations may be made to support qualified individuals with disabilities.
