Job description

The primary role of the Wound Care Specialist is to assess, manage, and treat patients with acute and chronic wounds across a variety of care settings including clinics, skilled nursing facilities, and home health environments.

Job Responsibilities

Key responsibilities include performing wound assessments, debridement procedures, and developing individualized treatment plans in collaboration with interdisciplinary care teams. The Wound Care Specialist should approach every patient encounter with clinical expertise, compassion, and integrity, ensuring evidence-based, timely, and effective care aligned with mission of improving outcomes and enhancing patient well-being.

1.Perform comprehensive physical examinations and wound evaluations on patients with acute and chronic conditions.

2.Assess wound-related conditions, review comorbidities, and coordinate treatment plans with the managing provider to support wound healing and overall health.

3.Perform wound debridement and dressing changes based on clinical necessity and best practice standards.

4.Develop and implement individualized care plans in collaboration with the Medical Director and interdisciplinary teams.

5.Accurately document all assessments, procedures, and communications in the EMR system.

6.Complete all clinical documentation within 24 hours of the patient encounter.

7.Provide patient and caregiver education regarding wound management, preventive care, and chronic condition support.

8.Communicate effectively with primary care providers, specialists, and interdisciplinary teams regarding changes in patient status.

9.Collaborate with medical assistants, clinical support staff, and regional leadership to maintain workflow efficiency.

10.Participate in clinical meetings, training sessions, and quality improvement initiatives.

Qualifications

Current, unrestricted Nurse Practitioner or Physician Assistant license in applicable state(s).

Clinical experience in wound care or a related specialty. Demonstrated proficiency in wound assessment, debridement, and plan-of-care development.

Wound care certification (WCC, CWCN, or equivalent). Experience working in SNFs, home health, or outpatient wound care settings. Familiarity with EMR platforms and mobile charting systems.
