We are seeking an experienced and compassionate Nurse Practitioner to join our Wound and Ostomy Care Clinic in Troy, NY. This is a full-time, weekday-only position with no weekend or on-call duties. The role involves both inpatient and outpatient responsibilities, working closely with surgical teams to support patients pre- and post-operatively.

Responsibilities:

• Provide assessments and develop individualized care plans for patients with ostomies or wound care needs

• Educate patients and families on ostomy care, post-surgical expectations, and lifestyle adjustments

• Collaborate with surgeons, nurses, and healthcare professionals for coordinated care

• Utilize evidence-based wound care techniques to support healing and prevent complications

• Maintain accurate documentation and ensure compliance with care protocols

Qualifications:

• Board Certified Nurse Practitioner (by start date)

• Active or eligible NY license

• WOCN Certification - Required

• DEA or willingness to obtain

• At least 1 year of NP experience, preferably in wound or ostomy care

• Strong communication skills and a passion for working with a vulnerable population

Local candidates within 30 miles of Troy, NY cannot be considered.
