Position Highlights:
- Nurse Practitioner or Physician Assistant opportunity in a Same Day Access Clinic
- Provide timely outpatient care for established patients requiring immediate clinical attention
- Focus on transitional care visits, hospital discharge follow-ups, and urgent outpatient needs
- Provide same-day clinical evaluation and treatment
- Evaluate, diagnose, and treat acute and subacute conditions
- Conduct transitional care management visits
- Coordinate patient care with primary care providers and multidisciplinary care teams
- Help improve access to care and support patients transitioning from the hospital to the outpatient setting
- Opportunity to help reduce hospital readmissions through timely follow-up care
- Patients scheduled continuously throughout the day
- 8-9 hour shifts
- Flexible scheduling options may be available
- Complete accurate and timely documentation in the EMR
- Fast-paced, collaborative, team-based clinical environment
Qualifications:
- Current Massachusetts Nurse Practitioner or Physician Assistant license
- Board certification required
- Primary care, urgent care, or transitional care experience preferred
- Strong clinical decision-making skills
- Excellent communication skills
- Ability to work efficiently in a dynamic same-day access environment
