Nurse Practitioner / Physician Assistant – Onsite Telehealth Urgent Care (Hybrid Model)
Location: Brooklyn, NY (Borough Park)
Employment Type: Permanent Full-Time or Permanent Part-Time (Per Diem Options Available)
Overview
A growing hybrid urgent care organization in Brooklyn, NY is seeking a Nurse Practitioner or Physician Assistant to provide telehealth urgent care services from the onsite clinical hub. While the care is delivered virtually, the provider must be physically present in the Brooklyn facility to perform video-based evaluations.
Medical assistants are dispatched to patient homes to conduct hands-on assessments, perform rapid tests, and use diagnostic tools (otoscope, digital stethoscope, video, audio). The NP/PA reviews the findings in real time, guides the MA, and manages the patient encounter via telehealth.
This is a permanent opportunity with flexibility for full-time, part-time, or consistent per diem scheduling.
Key Responsibilities

Deliver telehealth urgent care visits onsite from the Brooklyn location.

Collaborate with in-home medical assistants who perform physical assessments.

Diagnose and treat low-acuity conditions such as:

Flu, strep, COVID

Ear/eye infections
-
Rashes, URI symptoms

Basic medication refills
-

Review diagnostic inputs including vitals, rapid test results, audio recordings, and otoscope/stethoscope feeds.

Provide clear clinical guidance to medical assistants during visits.

Prescribe medications electronically and provide patient education.

Maintain accurate documentation and ensure proper follow-up care.
Schedule Options

Permanent Part-Time (e.g., 2–3 days/week)

Consistent Per Diem (example: guaranteed 15 hrs/week)
Client prefers ongoing, stable providers due to training and workflow consistency.
Requirements

Active NY NP or PA license
-
Experience in Urgent Care, Primary Care, ER, or Telehealth preferred

Must be comfortable working onsite in Brooklyn (not remote telehealth)

Strong communication skills for collaborating with in-home MAs

Ability to manage a high volume of low-acuity urgent care cases
