Responsibilities
- Conduct telemedicine consultations, assessments, and develop treatment plans for patients
- Provide education to patients on disease prevention, wellness, and chronic condition management
- Collaborate with physicians, specialists, and other healthcare professionals to ensure coordinated, high-quality care
- Maintain accurate and compliant patient records using electronic health systems
- Deliver compassionate, culturally competent care to diverse patient populations through telemedicine
Requirements
- Board-certified Nurse Practitioner with a valid New York State license
- Minimum of 2 years of clinical experience as a Nurse Practitioner
- Strong diagnostic and clinical assessment skills
- Proficiency with telemedicine platforms and electronic medical records
- Spanish language skills preferred but not required
