Practice Setting: Office-Clinic/Outpatient/Hospital Based/Rounding
Description of Ancillary Staff: 1 front office, 1 surgical scheduler, 1 onsite manager, 1 office RN, 2 Physician Assistants, 1 Nurse Practitioner, 2 Surgeons
Patient Age Demographic: Adult/Geriatric
Procedures/Surgeries Required: will not perform any procedures
Estimated Credentialing Timeframe: 90-120 days
Submission Requirements:
Board Certified (AGACNP - BC, ACNPC-BC, AGPCNP-BC, ACNPC-AG)
Active VA license & DEA
ACLS/BLS
