Job description JOB OVERVIEW
• Job Title: Advanced Practice Provider – Rheumatology
• Job Type: Permanent
• Location: Blackfoot, ID
• Service Setting: Outpatient Clinic within a Non-Profit Critical Access Hospital
• Schedule: Monday – Friday, standard daytime clinic hours

PATIENT POPULATION & SCOPE
• Patient Demographics: Adults
• Average Daily Volume / Census: Established outpatient rheumatology patient panel
• Admissions & Rounding: Not Included
• Consults & Follow-Ups: Yes
• Case Mix/Clinical Focus Areas: Chronic and complex rheumatologic conditions

FACILITY & TEAM DETAILS
• Beds in Department/Unit: Outpatient clinic setting
• Trauma Designation: N/A
• EMR System: In use at facility
• Specialty Backup: Dedicated Rheumatology physician support
• Support Staff: Experienced nursing staff and on-site Infusion Therapy Center team
• Leadership & Reporting Structure: Reports to Rheumatology Physician and Clinical Leadership
• Reason for Opening: Practice growth and increased patient demand

CREDENTIALING & COMPLIANCE
• Board Requirements: NP or PA certification in good standing
• DEA & Certifications: Active or eligible DEA; BLS required
• Credentialing Timeline: Standard permanent onboarding timeline

COMPENSATION & PERMANENT BENEFITS
• Base Salary: Nationally competitive salary

REQUIRED SKILLS & QUALIFICATIONS
• Board Status: Certified Nurse Practitioner or Physician Assistant
• State License Required: Active Idaho license or eligibility to obtain
• Certifications: BLS required
• Experience: Prior Rheumatology experience required
• Additional Requirements: Ability to manage an independent outpatient clinic panel and collaborate in a team-based environment

DUTIES & RESPONSIBILITIES
• Diagnose and manage chronic rheumatologic conditions in an outpatient setting
• Coordinate care with on-site Infusion Therapy Center
• Collaborate closely with Rheumatology physician and multidisciplinary care team
• Provide comprehensive follow-up care and patient education
• Maintain accurate and timely clinical documentation
