Physician Services Group (PSG) is seeking a Full-Time Nurse Practitioner to join our Post-Acute Care team in Aiken, SC. This is an excellent opportunity for both new graduates and experienced providers who are passionate about preventative care and improving long-term patient outcomes. You’ll work in a supportive, physician-led environment focused on building strong relationships and keeping patients out of the hospital. Enjoy a stable Monday–Friday schedule with no nights and weekends.
Job Responsibilities
Manage new admissions, follow-ups, and acute care visits
Coordinate care plans with facility teams, physicians, and specialists
Participate in discharge planning and ensure continuity of care
Document accurately using EMR systems and ensure E/M coding compliance
Serve as a clinical liaison and support facility compliance initiatives
Qualifications
Board-certified (ANCC or equivalent)
Strong communication, leadership, and clinical judgment
Experience in geriatric, hospital, or post-acute settings preferred
EMR proficiency and strong documentation skills
