Physician Services Group (PSG) is growing in the beautiful Lowcountry! We're seeking a compassionate and confident Nurse Practitioner (NP) to deliver exceptional care to residents in Post-Acute and Long-Term Care settings in Mount Pleasant, SC. This is a purpose-driven role ideal for a proactive provider who thrives in a team environment and values consistency, balance, and meaningful patient connections.
What You’ll Be Doing:
-Partner with an attending physician at the care facility
-Manage new admissions, routine Medicare follow-ups, and acute visits
-Coordinate care with specialists, therapists, and families
-Lead discharge planning efforts alongside social workers
-Ensure timely and accurate documentation using EMR systems
-Support facility compliance and quality initiatives
-Represent PSG with professionalism and compassion
What We’re Looking For:
-Nurse Practitioner certification (ANCC or equivalent)
-Active SC license (or ability to obtain)
-Solid clinical experience; acute or hospital background a plus
-Proficient in managing chronic and acute conditions
-Tech-savvy and comfortable with EMR systems
-Strong communication and self-management skills

Job Responsibilities
Perform comprehensive evaluations for new admissions, Medicare follow-ups, and acute needs
Collaborate with specialists, therapy teams, and families to coordinate treatment plans
Lead and document discharge planning in partnership with social workers
Utilize EMR systems for timely charting, coding, and billing
Ensure adherence to PSG care standards and facility compliance metrics
Serve as an on-site clinical leader, supporting ongoing quality improvement initiatives
Qualifications
South Carolina NP license (or eligibility)
Clinical background in acute, hospital, or post-acute settings preferred
Skilled in chronic disease management and acute intervention
EMR and medical documentation proficiency
Excellent communication, leadership, and time management skills
Working Hours
No weekends. No on-call. Day-shift stability with true work-life balance.
