NextGen Post Acute Care is a multi-specialty practice group specializing in pulmonary, infectious disease, cardiology, and post-acute care. We currently have over 100 providers delivering specialized care to patients in skilled nursing facilities across Florida, Ohio, Tennessee, and now in Pennsylvania and Texas.

A core aspect of our program is reducing hospital readmissions, ensuring responsible stewardship, and optimizing patient care within these facilities. Our Corporate Headquarters is in Hallandale Beach Florida.

We’re seeking a compassionate, experienced APRN to join our expanding team, providing patient-centered care across skilled nursing and long-term rehab settings. You’ll diagnose, treat, manage chronic and acute conditions, and collaborate with interdisciplinary teams across specialties including pulmonary, infectious disease, cardiovascular, pain management, psychiatry, and rehabilitation.

Job Responsibilities

Provide direct patient care in SNF/rehab/LTC settings
Perform assessments, physical exams, and develop treatment plans
Diagnose and manage acute and chronic conditions
Prescribe medications and follow clinical best practices
Document all care accurately in EMR/EHR
Conduct admissions and discharges
Collaborate with physicians, specialists, and facility staff
Support preventive care and educate patients/families
Contribute to reducing hospital readmissions
Maintain adherence to infection control and regulatory standards

Qualifications

Active TX APRN license
DEA registration (preferred)
BLS and ACLS certifications
Active NPI
Minimum of 1 years of nursing experience (Skilled Nursing preferred)
Knowledge of post-acute care and geriatric best practices
Strong communication, assessment, and leadership skills
Experience with EMR/EHR systems (Point Click Care/ Matrix)
Comfortable working independently in a fast-paced, clinical environment

Working Hours

5 days a week w/ weekend availability
