Schedule:
Wednesdays & Fridays
8:00 AM – 5:00 PM (1-hour lunch)
Position Summary
We are seeking a dedicated and experienced clinician to lead a multidisciplinary team in delivering high-quality care to patients with pulmonary conditions, such as COPD, asthma, and sleep apnea. This hybrid role includes in-person and telehealth care during standard business hours and is ideal for a provider passionate about innovation, patient engagement, and team-based care.
Key Responsibilities
Lead a multidisciplinary care team focused on pulmonary health.
Deliver direct patient care both in-clinic and via telehealth.
Develop, implement, and oversee individualized care plans.
Educate and empower patients on care models and available technologies.
Coordinate care with referring physicians and other healthcare providers.
Ensure high standards of documentation and EMR use.
Qualifications
Proven experience managing patients with pulmonary conditions (e.g., COPD, asthma, sleep apnea).
Passion for innovation and continuous quality improvement.
Ability to build, grow, and support new clinical models of care.
Excellent communication and interpersonal skills.
Experience in culturally competent, patient-centered care (including telehealth).
Skilled in mentoring and training both clinical and non-clinical staff.
Proficient with EMRs and digital tools in a healthcare setting.
Comfortable working in a fast-paced and evolving clinical environment.
