Community Health Services Nurse Practitioner / Physician Assistant (NP/PA)
Position Summary
The Community Health Services Nurse Practitioner or Physician Assistant provides comprehensive primary and preventive healthcare services to individuals and families within a community-based setting. This role focuses on improving access to care, managing acute and chronic conditions, promoting health equity, and delivering culturally competent, patient-centered care to diverse and underserved populations.
Essential Duties and Responsibilities

Provide comprehensive primary care services including health assessments, diagnosis, and treatment of acute and chronic conditions

Deliver preventive care services such as health screenings, immunizations, and wellness education

Manage chronic diseases including diabetes, hypertension, asthma, and cardiovascular conditions

Provide care coordination and referrals to specialty, behavioral health, and social support services
-
Address social determinants of health and connect patients with community resources

Promote health education, disease prevention, and lifestyle modification

Provide care across the lifespan as appropriate to scope of practice

Collaborate with physicians, nurses, care managers, social workers, and community partners

Maintain accurate, timely clinical documentation in the electronic health record

Participate in population health initiatives, outreach programs, and quality improvement activities

Support care transitions and continuity of care

Comply with all regulatory, accreditation, and professional standards
Qualifications

Graduate of an accredited Nurse Practitioner or Physician Assistant program
-
Current NP or PA certification and state licensure
-
Eligibility for credentialing and privileging

DEA registration or eligibility
-
Prior experience in community health, primary care, or public health preferred

Work Environment

Community health centers, outpatient clinics, mobile health units, or public health facilities

May include extended hours, outreach activities, or limited call depending on program needs
