Job description
We are seeking a compassionate, reliable, and skilled Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our team and provide on-site psychiatric care within long-term care and skilled nursing facilities. In this role, you will round regularly at assigned facilities, conduct psychiatric evaluations, manage ongoing mental health conditions, and prescribe medications as clinically indicated. Job Responsibilities
Develop and implement individualized treatment plans.
Prescribe medications as appropriate and monitor effectiveness and side effects.
Collaborate closely with facility staff, primary care providers, nursing teams, and families.
Maintain thorough and timely documentation in accordance with regulatory standards.
Participate in care plan meetings and interdisciplinary discussions as needed.
Provide compassionate, patient-centered care with a focus on improving quality of life.
Qualifications
National certification as a Psychiatric Mental Health Nurse Practitioner
DEA registration
Experience in geriatrics, long-term care, or medically complex populations (preferred but not required)
Strong clinical judgment and ability to work independently
Excellent communication and collaboration skills

Conduct thorough psychiatric assessments for geriatric and complex patients
Manage medications with attention to psychopharmacology and side effects

Geriatric & Long-Term Care Competence:
Understand dementia behaviors and common psychiatric conditions in older adults
Navigate long-term care regulations and documentation standards
Manage chronic mental health conditions alongside medical comorbidities
Communication & Collaboration:
Communicate clearly with patients, families, and facility staff
Collaborate effectively within interdisciplinary teams
Provide education and guidance on diagnoses and care plans
Autonomy & Time Management:
Work independently with minimal supervision
Manage time efficiently across multiple facility sites
Maintain consistent follow-up and timely documentation
Patient-Centered & Trauma-Informed Care:
Demonstrate empathy and cultural sensitivity
Use de-escalation and behavior management techniques
Provide compassionate care to vulnerable populations
Documentation & Technology:
Document accurately and efficiently in EHR systems
Understand regulatory and compliance requirements
Utilize telepsychiatry tools when applicable
