Low Country Health Care System, a CIMS Network Facility, is seeking a Psychiatric Mental Health Nurse Practitioner to join its team in Barnwell, SC. The Mid-Level Provider shall provide professional, psychiatric care and treatment services to patients across the lifespan, as well as perform all professional duties in accordance with policies and procedures of Low Country Health Care System, Inc. The Mid-Level performs these functions under the supervision of a licensed physician in the primary health care setting.

Job Responsibilities

The RCORP-Overdoes Response project will support improved health care in rural Allendale, Bamberg, Barnwell, Calhoun, and Orangeburg counties by addressing immediate and short-term needs related to the provision of SUD (Substance Use Disorder) services, and will reduce and prevent the risk of overdoses. Behavioral Health disorders, including SUD, opioid use disorders (OUD), and mental illnesses including depression and anxiety are a significant and growing concern throughout the service area. PMHNP will see all patients who screen positive for SUD/OUD and related infectious diseases connected with treatment, including MOUD.

 Assess, diagnose, and treat adults, adolescents, and children experiencing symptoms of mental health disorders. This includes providing individual therapy as appropriate and prescribing medications as indicated.
 Consult and collaborate with the psychiatrist, development/behavioral health pediatrician, social workers, and counselors to provide a team approach and collaborative care model.
 Attend Mental Health Group meetings and collaborate with the Mental Health Group to promote positive patient outcomes.
 Provide telehealth and televideo services as needed.
 Examines patients if needed for psychiatric assessment.
 Provide emergency medical care to patients when necessary.
 Prepare progress notes, summaries and general evaluations of patient records; consults with relatives and/or caregivers of the patient regarding the patient’s physical condition.
 Consult other internal and external providers to provide medical care, psychiatric care, and care coordination
 Order routine and diagnostic labs and other necessary tests as indicated and interpret results.
 Develops and implements patient comprehensive management plans, records in EMR progress notes
 Collaborates with the health team members to establish priorities, manage care and coordinate efforts to achieve desired outcomes.
 Provide an after visit summary review for patients whenever appropriate.
 Educate patients about self-management tasks.
 Assess barriers to meeting treatment goals and advise patient care team of impacts to plan of care.
 Utilize EMR in documenting all clinical data.
 Ensure care coordination, referral management, clinical workflow processes by meeting regularly with clinical staff.
 Provide clinical care using team based approach centered on patient needs and incorporating individual patient preferences in the delivery and approach to care.
 Attend all staff meetings, lectures, seminars and case presentations.
 Assume an active membership role in national, state and local medical societies.
 Establish a rapport with public and private clinical care referral resources. Participate in community outreach as needed.
 Provide practice call as per contract negotiations.
 Participate in quarterly chart reviews and on-going Health Care Collaborative as requested.
 Maintain protocols specific to job as mandated by medical board.
 Participate in other activities, including but not limited to health fairs and responding to emergencies as needed.
 Perform any other duties as assigned by your supervisor.

Qualifications

Completion of an accredited, formal education program. Must maintain active certification as a PMHNP by the ANCC. Active license with the state of South Carolina, including prescriptive privileges with appropriate scheduled drugs approved through the Drug Enforcement Agency. Maintenance of Basic Life Support techniques, at a minimum with Advanced Cardiac Life Support preferred.

Working Hours

Full-Time
Monday, Tuesday each week at Bamberg/Orangeburg locations.
Wednesday, Thursday, Friday at BFM.
1 day per month will work on the Mobile Medical Unit.

5 days a week or 4 days per week can be negotiated.

Skills

Skill set will be agreed upon with supervisory physician within scope of his/her practice.
