Psychiatric Nurse Practitioner

Location: Pittsburgh, PA | Setting: In-Office | Patient Load: 10–12 per day

This is not your typical psychiatry role.

AbsoluteCare is seeking a compassionate, mission-driven Psychiatric Nurse Practitioner to bring the principles of team-based, collaborative care and population health to life for our medically complex and socially vulnerable members. Our members often face significant barriers to care and have been underserved by traditional healthcare systems—frequently cycling through emergency rooms, hospitals, and specialist visits without receiving the coordinated support they need.

We break that cycle by delivering comprehensive, integrated care that improves quality of life and health outcomes while reducing unnecessary utilization. Our care teams include primary care providers, complex care managers, nurses, medical assistants, behavioral health clinicians, and community health workers. Together, they provide wraparound services supported by onsite resources such as an infusion center, full-service pharmacy, and lab and imaging suites. We also address critical social determinants of health, including housing instability, food insecurity, and social isolation.

In this role, the Team Psychiatric Provider serves as a psychiatric subject matter expert within the interdisciplinary team and works closely with colleagues to enhance the psychiatric care of our members. This includes managing a defined cohort of patients through a modified Collaborative Care Model, supporting behavioral health outcomes through case reviews and team consultations, participating in regular care team huddles, developing targeted educational materials for staff, and providing direct psychiatric care to our most vulnerable patients.

Job Responsibilities

As a Team Psychiatric Provider, you will collaborate with interdisciplinary teams to improve behavioral health outcomes for our members. Your responsibilities will include:

Clinical Leadership and Collaboration
Guide behavioral health treatment for a defined patient cohort using a modified Collaborative Care Model. Participate in daily team huddles and case reviews to support high-risk, high-utilizing members. Provide consultative support and education to providers on psychiatric and substance use disorder management.

Direct Patient Care
Deliver in-person and tele-psychiatry services, including psychiatric evaluations, medication management, and treatment planning. Conduct post-discharge visits and support transitional care for patients leaving psychiatric facilities. Support crisis intervention, emergency department diversion, and medication reconciliation, including long-acting injectables (LAIs).

Addiction Medicine and MAT
Support ambulatory-based addiction treatment using medications such as buprenorphine and naltrexone. Help develop clinical protocols for Medication-Assisted Treatment (MAT). Provide direct care for patients with substance use disorders and assist in transitions to higher levels of care when needed.

Qualifications

Licensed and Board-Certified Psychiatric Nurse Practitioner in Pennsylvania
3–5 years of relevant experience as an Advanced Practice Provider
DEA license and BLS certification
Strong EMR skills and comfort with technology
Passion for serving underserved populations
Collaborative, team-first mindset with excellent communication skills

Preferred:

Experience in addiction medicine and population health
Background working with medically and psycho-socially complex Medicaid beneficiaries
