Psychiatric Nurse Practitioner – Community Mental Health (Shelter-Based)

Annual Salary: $135k-$185k (varies depending on years of experience)

A community-focused behavioral health organization is seeking a dedicated Psychiatric Nurse Practitioner to provide psychiatric care and medication management to adult men residing in transitional housing.

Key Responsibilities:

• Conduct psychiatric evaluations and manage medication regimens for adult male clients with complex behavioral health needs.
• Support treatment adherence and coordinate care, including follow-up appointments and referrals.
• Collaborate with shelter staff and case managers to ensure continuity of care and crisis intervention.
• Provide hands-on support to clients, including motivational engagement and psychoeducation.
• Maintain accurate documentation and contribute to care planning in coordination with a collaborating psychiatrist.
• Travel between two fixed shelter locations weekly; mileage and tolls reimbursed.

Requirements:

• Licensed Psychiatric Nurse Practitioner (PMHNP) in New York State.
• Minimum of 2 years’ experience working with adults with serious mental illness.
• Experience in shelter-based care, ACT/IMT teams, or similar community mental health settings strongly preferred.
• Must have access to a personal vehicle and be comfortable traveling between two boroughs.
• Strong interpersonal skills and a commitment to working with underserved populations.
