Health Care Partners of South Carolina, a CIMS network center, is seeking a Psychiatric Mental Health Nurse Practitioner to join its team! The Psychiatric Mental Health Nurse Practitioner (PMHNP) will provide outpatient psychiatric evaluations and medication management services at Healthcare Partners of South Carolina, Inc. The PMHNP’s function is to evaluate clients on the need for
psychopharmacologic interventions and subsequently monitor clients to assess symptom
management, side effects and efficacy of prescribed medications. Upon prescribing
psychopharmacologic medications, the PMHNP will provide counseling/education to clients
regarding compliance, dosing, side effects and any other necessary education that will benefit
the client. This position requires a high degree of self-direction and the ability to work
independently. The PMHNP must possess excellent interpersonal skills, the ability to
collaborate and communicate effectively in a team setting and the ability to maintain
effective and professional relationships with patients and other members of the care
team.

Job Responsibilities

• Provide thorough initial comprehensive evaluations, diagnosis, and treatment of
patients.
• Form a plan of care including prescribing and administering appropriate
medications, providing brief supportive therapy, providing referrals to other
disciplines, and acting as liaison for patients with other health care providers,
guardians, and their families.
• Maintain complete, accurate, timely and confidential documents in the electronic
health record.
• Establish therapeutic relationships with patients to add psychiatric expertise to the
medical management plan and mitigate any negative impact that behavioral
conditions can have on patients’ overall health and well-being.
• When appropriate, communicate with the patient’s primary care provider
regarding changes in patient condition.
• Maintain partnership with supervisor through regular communication, care plan
development, medication management, etc.
• Participate in quality assurance, educational, and professional meetings as
assigned.
Qualifications
• Master’s degree or higher in Advanced Practice Registered Nursing (APRN)
• Current state Nurse Practitioner license with prescriptive authority.
• Current and unencumbered DEA
• DATA 2000 waiver preferred
• CPR Certified
