Must be PMHNP certifiedConnect Care provides health care services to consumers into their homes or other remote locations in the Intermountain region via telehealth modalities (video, e-visits, phone, messaging, etc.). The Connect Care Behavioral Health practice offers care 7 days per week, 7am - 7pm, for mild to moderate mental health diagnoses with medication checks, refills, adherence, and management.
This position will support the healthcare workforce.
This NP will work four 10-hour shifts, Saturday, Sunday and two weekdays of their choice with at least one shift working until 7 p.m.
Psychiatric Nurse Practitioner Connect Care
Advanced Practice Providers (APPs) are accountable for the provision of patient care at the top of licensure, within their scope of practice. Licensed, credentialed and privileged to perform such duties as:
Maintenance and promotion of health and prevention of disease;
Assessment, diagnosis, treatment, correction, and consultation;
Procedures as outlined by specialty, and referral for health problems;
Education of the patient, patient's family, clinical staff, and others;
Prescription or administration of prescription drugs or devices in accordance with a consultation and referral plan;
Consultation with other healthcare providers;
Appropriate and timely documentation of clinical care;
Continually learning and developing for optimal patient care and outcomes.
Advanced Practice Providers may also participate in research, utilize evidence-based practice to improve quality patient care, protocol development, care process model development, and best practice standards for assigned patient population throughout our organization.
