We seek a Nurse Practitioner / Physician Assistant to help conduct comprehensive psychiatric evaluations and develop treatment plans for patients in post-acute care facilities. Our ideal candidate will have a vast knowledge of psychotherapy modalities, psychopharmacology and behavioral healthcare relating to geriatric patients. This position will have the autonomy of assessing, diagnosing, and developing regimented therapy and treatment plans in collaboration with our supervising physician.

DUTIES AND RESPONSIBILITIES include, but are not limited to,

  • Conducting comprehensive psychiatric evaluations of patients.
  • Providing psychiatric health services, education, counseling and emotional support.
  • Developing and implementing treatment plans based on each patient's needs and diagnoses
  • Provide medication management and prescribe appropriate medications.
  • Collaborate with interdisciplinary healthcare team members to ensure coordinated and integrated care.
  • Effectively coach and educate patients and their families about psychiatric conditions, treatment options, and self-care strategies.

ESSENTIAL ABILITIES

  • Well-developed problem-solving skills that help identify and interpret key risk factors, such as medical and family history of mental illness, trauma, substance abuse, and environment, in conjunction with medical history and other record findings.
  • Ability to ascertain necessary laboratory and diagnostic procedures needed in order to interpret data and determine preliminary diagnosis and therapeutic plans.
  • Ability to communicate with respect and tact when communicating with patients and their families and evaluating a patient’s mental health.
  • Skilled in establishing connections that encourage patient and family participation.
  • Maintain professionalism and organizational values when acting as an advocate for those coping with mental illness
  • Effectively provide both telehealth and onsite visits.
  • Be able to commit to work a minimum of 10 hours per week, between the hours of 9am-5pm Monday through Friday, 2 or more hours time block each time.

QUALIFICATIONS

  • Master's degree in Nursing Licensure, Certification, and Registration
  • Current, valid Florida APRN license Certification through the ANCC, AANP (or equivalent certification body)
  • Current BLS required Medicare NPI #, 855I and 855r on file (Independent NP Medicare Provider Number/PTAN#)
  • Must be able to commit to work a minimum of 10 hours per week, between the hours of 9am-5pm Monday through Friday.(in 2 or more hours time block each time)
  • No weekends or evenings at this time.
  • Psychiatric-focused clinical experience (3+ years)
  • Geriatrics and telehealth experience are highly preferred.

Our clinician-focused technology and resources will help you:

  • Conduct clinical assessments and evaluations.
  • Formulate treatment plans as needed.
  • Prescribe medications with proper dosage and administration instructions.
  • Oversee telemedicine patient flow and daily functions of the telemedicine shift.
  • Maintain accurate electronic medical records of patients' contact details, medical history, prescribed medications, allergies, diagnoses, and progress.
  • Maintain integrity, availability, and confidentiality of all PHI
  • Oversee the reporting of diagnostic testing results, follow-up treatment, referrals, and recovery.
  • Participate in quality assurance and care improvement initiatives.
  • Adhere to HIPAA standards for information security, privacy, and confidentiality.
