We seek a Nurse Practitioner / Physician Assistant with strong clinical Physical Medicine & Rehabilitation experience to help provide services to our post-acute patients and provide follow-up telehealth visits, as needed. As part of an interdisciplinary care team, this position will provide telehealth and onsite visits as part of ongoing medical management and acute interventions for patients in the the post-acute care setting. DUTIES AND RESPONSIBILITIES, may include but not be limited to

  • Diagnosis and treatment of patients experiencing limitations in function due to any disease process, injury, or symptom.
  • Coordinate the patient plan of care with the physician and the facility's clinical disciplines to address patient needs, evaluate the effectiveness of care, and assess changes to the patient medical condition to ensure patient centered rehabilitation goals are being met.
  • Establish plan of care and work within protocols established by the supervising physician.
  • Order diagnostic tests and studies and prescribe medications as needed.

ESSENTIAL ABILITIES

  • Detailed knowledge and experience in Physiatry that works to restore function and improve quality of life for a person who has been disabled as a result of a disease, disorder, or injury
  • Ability to help diagnose, treat, and/or manage a wide range of physical disabilities and chronic conditions
  • Ability to provide both telehealth and onsite visits to skilled nursing facilities.
  • Must be able to commit to work a minimum of 10 hours per week, between the hours of 9am-5pm Monday through Friday, 2 or more hours time block each time
  • Excellent communication skills and ability to effectively communicate with patients, family members, and other medical professionals.
  • Comfortable working independently and in collaboration with other medical team members in utilizing approved protocols, procedures, and expert consultation for our geriatric patients.

QUALIFICATIONS

  • Master's degree in Nursing Licensure, Certification, and Registration
  • Current, valid Florida APRN license Certification through the ANCC, AANP (or equivalent certification body)
  • Current BLS required
  • Medicare NPI #, 855I and 855r on file (Independent NP Medicare Provider Number/PTAN#)
  • Must be able to commit to work a minimum of 10 hours per week, between the hours of 9am-5pm Monday through Friday.(in 2 or more hours time block each time)
  • No weekends or evenings at this time.
  • Physical Medicine & Rehabilitation-focused clinical experience (3+ years required)
  • A passion for post-acute healthcare and serving the geriatric population.
  • Geriatrics and telehealth experience are highly preferred.

Our clinician-focused technology and resources will help you:

  • Conduct clinical assessments and evaluations.
  • Formulate treatment plans as needed.
  • Prescribe medications with proper dosage and administration instructions.
  • Oversee telemedicine patient flow and daily functions of the telemedicine shift.
  • Maintain accurate electronic medical records of patients' contact details, medical history, prescribed medications, allergies, diagnoses, and progress.
  • Maintain integrity, availability, and confidentiality of all PHI
  • Oversee the reporting of diagnostic testing results, follow-up treatment, referrals, and recovery.
  • Participate in quality assurance and care improvement initiatives.
  • Adhere to HIPAA standards for information security, privacy, and confidentiality.
