| Details of Assignment: | Primary care patient panel for 0-18 years. 18 patients per day Chart closure within 72 hours. Travel and lodging are pass through. 30 day notice required for ending assignment early. Ongoing assignment but must agree to 3-6 month minimum. |

| Minimum Experience and Certification Required: | Pediatric experience required. Can be NP without peds BC but must have extensive pediatric experience to be consider. Preferred prior FQHC experience. Must have prior experience treating children with BH(anxiety) issues. Must have pediatric Chronic care Management. Active DEA. BLS, Flu, TB, MMR, Hep B, Tdap, Varicella all required titers, vaccines, or declinations. |
