Job description
We are seeking an experienced Nurse Practitioner (NP) to join a multidisciplinary team in a pediatric subacute care environment. The NP will provide comprehensive direct patient care, collaborate with an interdisciplinary care team, and support the overall well-being and recovery of medically complex children from birth to 21 years old. This role focuses on individualized treatment planning, functional assessments, and ongoing evaluation to ensure the highest quality of care.
Job Responsibilities
Perform comprehensive functional assessments based on patient age, medical history, and presenting condition.
Conduct patient interviews, health histories, physical/functional exams, and interpret diagnostic tests.
Manage patient care, including assessment, developing diagnostic plans, and following established medical protocols.
Refer patients to appropriate clinics, subspecialties, and community resources as needed.
Collaborate with physicians as a consultant on patient care plans.
Prepare, administer, and evaluate individualized health care and treatment plans.
Prescribe medications safely and effectively in accordance with state regulations.
Provide patient and family education related to medical and functional health status.
Determine the effectiveness of care plans, document outcomes, and modify plans as needed.
Participate in patient care reviews, seminars, and team education initiatives as requested by leadership.
Qualifications
Master's degree in Nursing / Nurse Practitioner program required.
Experience:
Minimum 1 year of nursing experience required.
At least 1 year of Nurse Practitioner experience preferred.
Minimum 2 years of pediatric critical care or ER experience as an RN.
Licenses & Certifications:
Current state Nurse Practitioner license.
Current state RN license.
Nurse Practitioner certification in the relevant specialty area.
BLS certification.
