🩺 Key Responsibilities

• Conduct routine physical exams, diagnose, and treat patients

• Manage chronic conditions and implement care plans

• Order, review, and interpret lab and imaging tests

• Prescribe and adjust medications as needed

• Provide preventive care, including vaccinations

• Document clinical encounters accurately in EMR/EHR

• Collaborate with facility staff and consult specialists when needed

• Deliver compassionate care to nursing home residents

✅ Required Qualifications

• Master’s Degree in Nurse Practitioner Program (Family or Adult-Geriatrics)

• Active New York State NP License

• National Board Certification (AANP or ANCC)

• Strong clinical judgment and patient care skills

• EMR/EHR experience preferred

• Bilingual (Spanish) a plus
