Schedule: Monday – Friday, 8:00am – 5:00pm (40 hours/week)

Position Overview

We are seeking a dedicated Nurse Practitioner to join a community health organization in San Jose, CA. The provider will work in a school-based setting serving both students and members of the local community. This is a full-time, Monday through Friday role focused on providing primary care services to patients of all ages.

Responsibilities

• Provide comprehensive primary care to patients across the lifespan.

• Support chronic disease management and assist in meeting HEDIS quality measures.

• Collaborate with multidisciplinary staff to ensure high-quality patient care.

• Maintain accurate and timely charting using Epic EMR.

• Participate in a full week of mandatory orientation prior to starting.

Details

• Patient Volume: ~21 patients per day (21+ preferred)

• Patient Population: All ages (pediatric through adult)

• Setting: Outpatient / school-based primary care

• Support Staff: On-site clinical and administrative support

• Orientation: One full week (Monday–Friday, required attendance)

• Credentialing Timeframe: 30 days or less

Requirements

• Active California NP license (required)

• Board Certified Nurse Practitioner

• BLS certification (required)

• Experience with Epic EMR preferred

• Must be comfortable working with children and adults

• Willingness to commit to the full 6-month contract
