Adventist Health St. Helena is looking for an Advanced Practice Genetics Nurse Practitioner who will be responsible for providing cancer risk assessment and cancer genetic services to patients referred to the AHEAD program. Services include: cancer risk consultation, genetic risk evaluation, cancer genetic counseling, facilitation of genetic/genomic testing, determination of management recommendations, communication with referring physician(s) and assistance with enrollment in research studies, registries and clinical trials. The APNG will work with Medical Directors of the AHEAD program to define the scope of practice and the process, implementation and evaluation of services.

Minimum Requirements:

Education: Master's Degree in Nursing-required

Licensing: Active status, California Board of Registered Nursing-required

Certification/Credentialing:

Nurse Practitioner-Board Certified, California Board of Registered Nursing-required

Credentialing as Advanced Practice Nurse in Genetics (APNG)-required


Experience:

• Minimum of three years in cancer risk assessment, cancer genetics/genomics
• Provision of cancer risk services according to scope and standards of genetics clinical nursing practice
• Extensive experience with cancer risk assessment models
• Strong oncology and cancer prevention background
• Incorporation of recent literature and research into clinical practice
• Utilization of automated risk assessment software

Responsibilities:
• Evaluates, educates and counsels patients and families at risk of hereditary cancer predisposition
• Utilizes appropriate models for calculating an individualized cancer risk assessment including prior probability of an inherited genetic mutation
• Ensures systematic collection of risk factors, detailed family history and 3-4 generation pedigree construction
• Retrieves and reviews selected medical records
• Assesses appropriateness of germline and somatic genetic testing including polygenic risk. Assesses need for other tests or examinations as needed

• Obtains informed consent discussion prior to genetic/genomic testing
• Facilitates germline and somatic testing as appropriate; counsels patients following test result(s).
• Utilizing expert society guidelines, develops individualized management plan in collaboration with patient
• Educates patient as to the importance of cascade testing
• Generates consultation and follow-up notes
• Works with Medical Director(s) to establish protocols for referral and follow-up care
