- Reason for Coverage: Staff Leveling
- Start Date: 9/11/2023 – Depends on Credentialing Time Frame & When Provider is leaving
- End Date: 1/12/24
- Work Schedule: M - F 8a - 4p For the Clinic – Evening Call NO
- Staff Information: Director, 1 APP, Nurse Manager
- Patient Information: 12 -14 PPD – Adult Population Follow-ups
- Required Procedures/Skills: 25% - Hematology & 75% - Medical Oncology – IP Consults
