Job Responsibilities
• Develop care plans to slow disease progression and improve outcomes
• Conduct dialysis rounds (in-center & home modalities)
• Manage electrolyte imbalances and medication therapies
• Coordinate care across clinic, hospital, and dialysis settings
• Educate patients on kidney health, dialysis options, and transplant readiness
• Collaborate with nephrologists, dietitians, and care teams
• Maintain accurate EMR documentation
Qualifications
• Active or eligible NP license
• National certification (AANP/ANCC)
• DEA license (or eligibility)
• Strong clinical and patient communication skills
• New grads welcome (training/support provided)
