Your Role

As a Nephrology Nurse Practitioner, you will:

• Evaluate and manage patients with CKD, ESRD, hypertension, electrolyte disorders, and other renal conditions 🧠
• Develop proactive care plans to slow disease progression and improve outcomes
• Manage medication optimization and chronic disease care
• Provide continuity of care across clinic, hospital, and dialysis settings 🏥
• Conduct dialysis rounds (in-center and home modalities)
• Manage vascular access and dialysis-related complications
• Support transplant education and dialysis modality counseling

What We’re Looking For

• Master’s or Doctorate in Nursing (MSN or DNP)
• Active or eligible Nurse Practitioner license in New Mexico
• National board certification (AANP, ANCC, or equivalent)
• DEA license with prescriptive authority
• Strong clinical judgment and patient-centered mindset ❤️
• Interest or experience in nephrology is highly valued
