A well-established physician-owned, multi-specialty medical group in the scenic Hudson Valley is seeking an experienced Nephrology Nurse Practitioner to join its collaborative care team. Job Responsibilities
β’ Perform inpatient nephrology consultations and hospital rounding
β’ Conduct daily dialysis rounding and manage acute and chronic dialysis patients
β’ Order and interpret diagnostic testing including laboratory and imaging studies
β’ Collaborate with nephrologists, dialysis staff, and multidisciplinary healthcare teams
β’ Develop and implement comprehensive treatment plans for patients with kidney disease
β’ Educate patients and families regarding disease management, treatment options, and lifestyle modifications
β’ Maintain accurate and timely medical documentation
β’ Participate in clinical quality improvement initiatives
Qualifications
β’ Current New York State Nurse Practitioner license
β’ National certification (ANCC or AANP)
β’ Masterβs or Doctoral degree in Nursing
β’ Valid DEA license
β’ Minimum 2 years of nephrology experience
β’ Experience providing both outpatient and inpatient nephrology care
β’ Knowledge of dialysis management and renal procedures
β’ Strong clinical assessment and diagnostic skills
β’ Ability to work independently and collaboratively within a healthcare team
Preferred
β’ Experience with hospital rounding and dialysis management
β’ Familiarity with electronic medical record (EMR) systems
β’ BLS and ACLS certification
