Soledad, California, Nephrology Nurse Practitioner Job – Post Transplant

Position Overview

  • Title: Nephrology Nurse Practitioner – Post-Transplant Care

  • Location: Soledad, CA (clinic, in-home, occasional telehealth)

  • Schedule: Full-time | Monday–Friday | 8am–5pm PST

  • Setting: Field-based (mostly in-clinic and in-home)

  • Reason for Need: Practice growth

Clinical Responsibilities

  • Care for kidney transplant recipients 1+ year post-transplant

  • Focus: Chronic disease management (HTN, DM, CKD, cardiovascular risk)

  • Review labs, monitor transplant function, adjust medications (within scope)

  • Coordinate care, referrals, and follow-up

  • Educate patients on medications, lifestyle, and routine maintenance

  • Participate in interdisciplinary team (IDT) meetings and quality initiatives

  • Perform Annual Wellness Visits and Comprehensive Health Evaluations

  • Conduct in-home visits within local region

Requirements

  • License: Active California NP license & DEA (or willing to obtain)

  • Certifications: AANP/ANCC certified, BLS

  • Experience: Minimum 2 years in transplant, nephrology, or complex chronic disease

  • Knowledge of immunosuppressants, transplant protocols preferred

  • California driver’s license required

  • Preferred: Bilingual Spanish, EMR/transplant workflow experience

Compensation & Benefits

  • Salary: $145,000–$160,000/year
