Position Highlights
- Full-time Post-Acute Care opportunity
- 4-day-per-week schedule
- Round in skilled nursing facilities
- Provide comprehensive care to a diverse post-acute patient population
- Perform medical histories, physical exams, and comprehensive patient assessments
- Develop and adjust individualized treatment plans
- Monitor patient progress and coordinate care to optimize outcomes
- Collaborate with physicians, nursing staff, and multidisciplinary care teams
- Educate patients and families on health management and preventive care

Qualifications
- Nurse Practitioner or Physician Assistant
- Active New Mexico NP or PA license
- Active DEA registration
- Post-acute, acute care, Emergency Medicine, or outpatient clinical experience preferred but not required
- Strong clinical and communication skills
- Collaborative, team-oriented approach
