Seeking Nurse Practitioner or Physician Assistant to join post-acute care team in Hartford, Connecticut
Position Highlights:

Responsibilities:
- Perform comprehensive patient assessments, including histories and physical examinations
- Develop, implement, and adjust individualized care plans
- Monitor patient progress and coordinate care across disciplines
- Collaborate with physicians, nursing staff, therapists, and other healthcare professionals
- Educate patients and families regarding disease management and preventive care
- Maintain accurate, timely, and compliant medical documentation
- Deliver high-quality, patient-centered post-acute care
Qualifications:
- Nurse Practitioner or Physician Assistant certification
- Active Connecticut NP or PA license
- Active DEA or ability to obtain
- Experience in post-acute, acute care, emergency medicine, or outpatient practice preferred
- Strong communication, clinical assessment, and teamwork skills
Compensation & Benefits:
- Estimated compensation range: $158,000 - $188,000 annually
