Primary Care Advanced Practice Provider
Nurse Practitioner (NP) or Physician Assistant (PA-C)
Greater Sacramento Area, CA (Multiple Locations)
Compensation: $131,000 – $170,000 annually (based on experience)
Full-Time | Monday–Friday 8:00 AM – 5:00 PM or 4-Day (10-Hour) Schedule
We are seeking an experienced Nurse Practitioner or Physician Assistant to join a large, mission-driven community health organization serving diverse and underserved populations throughout the Greater Sacramento region. With approximately 60 providers currently on staff, this is a stable, collaborative, team-based outpatient environment.
This role is focused exclusively on outpatient primary care within an FQHC/community health setting.
Productivity & Clinical Expectations
Providers are expected to see approximately 3 patients per hour
Strong charting efficiency and in-basket management skills required
EPIC EMR experience strongly preferred
Comfort managing a structured, productivity-driven outpatient schedule
Experience Requirements
Minimum 1–2 years of FQHC or community health outpatient primary care experience required
Must have experience treating diverse and underserved patient populations
Not ideal for new graduates
On-Call
On-call participation is required
Frequency varies depending on provider type and clinic location
Position Overview
The Advanced Practice Provider will deliver comprehensive primary care services within scope of practice while collaborating closely with physicians, medical leadership, and multidisciplinary care teams.
Key Responsibilities
Perform comprehensive history and physical examinations
Diagnose and manage acute and chronic conditions
Order and interpret labs, imaging, and diagnostic testing
Prescribe medications within scope of practice
Develop and implement individualized care plans
Monitor patient progress and adjust treatment plans as needed
Provide preventive care, health education, and counseling
Initiate emergency interventions when clinically indicated
Maintain accurate, timely documentation in the electronic health record
Collaborate with physicians and care teams to ensure coordinated patient management
Qualifications
Active California NP or PA license
Current board certification (NP or PA-C)
Current DEA registration
Current BLS certification
Minimum 1–2 years outpatient community health or FQHC experience
Strong commitment to serving low-income, homeless, or medically underserved communities
Bilingual in English and Russian, Farsi, Pashto/Pushto, or Spanish strongly preferred
Compensation & Benefits
Salary range: $131,000 – $170,000 based on experience
