Job description Here We Grow Again. Physician Services Group (PSG) is seeking a Full-Time Nurse Practitioner (NP) to join our Post-Acute Care team in the Greenville/Pickens, South Carolina area. This role offers a consistent Monday–Friday schedule, clinical autonomy, and a collaborative team-based environment focused on high-quality, patient-centered care. The NP will provide in-person care within a Skilled Nursing/Post-Acute Care setting and work closely with attending physicians and on-site clinical teams to reduce hospital readmissions and support long-term patient outcomes.

Job Responsibilities

Evaluate and treat new admissions, follow-ups, and long-term care patients

Serve as a primary provider in the post-acute/skilled nursing setting

Deliver patient care in accordance with South Carolina Nurse Practice Act and PSG guidelines

Collaborate with attending physicians, nursing staff, and interdisciplinary teams

Manage chronic conditions and provide preventative and geriatric-focused care

Ensure accurate, timely documentation in the EMR

Communicate treatment plans and updates with patients, families, and facility staff

Qualifications

Active Nurse Practitioner license in South Carolina (or ability to obtain)

Board Certified (ANCC, AANP, or equivalent)

Strong clinical judgment and documentation skills

Experience in post-acute, hospital, or outpatient care preferred

Commitment to high-quality, patient-centered care

Working Hours

Full-Time

Monday–Friday

8:00 AM – 4:00 PM

In-person, on-site at a Skilled Nursing/Post-Acute Care facility
