About the Role:
As a Nurse Practitioner in post-acute care, you will provide in-person care to patients in skilled nursing and rehabilitation facilities. In this role, you will partner with facility teams and Trumen-affiliated physicians to manage chronic and acute conditions, support transitional care planning, and contribute to quality-improvement initiatives within your designed facilities.

Job Responsibilities
Collaborate with Trumen physicians and facility medical directors to coordinate patient care
Conduct comprehensive clinical assessments and manage treatment plans
Support transitional care planning to help prevent avoidable hospital readmissions
Reconcile medications and provide patient education consistent with scope of practice
Foster positive working relationships with facility leadership, clinical staff, and care coordinators
Collaborate as applicable Trumen-affiliated value-based and quality improvement initiatives
Serve as a liaison between Trumen and disignated skilled nursing facilities (SNFs)
Maintain accurate and timely documentation within the EMR
Maintain an on-site presence consistent with your agreed schedule and facility needs
Qualifications
Active Nurse Practitioner license in the state of Texas
Minimum of 1 year experience as a Nurse Practitioner, preferably in a post-acute, SNF, or geriatric care setting
Ability to work independently while effectively collaborating with the care team
Strong interpersonal skills and a passion for caring for complex patients
