Responsibilities and Duties
- Deliver basic medical services to adult patients aged 19 to 70s
- Manage a patient panel primarily at nursing home level acuity, including individuals with dementia, distal muscular dystrophy, and traumatic brain injury
- Provide care in a facility licensed as an LTACH but functioning as a standard long-term care or skilled nursing facility
- Collaborate with interdisciplinary teams to ensure optimal patient outcomes
- Maintain accurate and timely documentation of patient care
- Adhere to all facility protocols and care standards
Additional Information
- Rhode Island license required
- Board certification in Family Medicine or Internal Medicine required
- Experience with long-term care or similar patient populations preferred
