Nurse Practitioner (Part-Time) – Post-Acute & Rehab Care | Physician Services Group (PSG)

Location: South Carolina (In-Person)
Schedule: Monday–Friday, 8:00 AM – 4:00 PM | 3 days per week

About the Opportunity

Physician Services Group (PSG) of South Carolina is seeking a part-time Nurse Practitioner who is passionate about providing quality, compassionate care in a Skilled Nursing, Assisted Living, and Rehab setting. You’ll work side-by-side with an attending MD, managing post-acute and long-term care patients while helping them achieve the best possible outcomes.

This is a hands-on role ideal for a provider looking to make a meaningful impact in the post-acute space and grow within a supportive, physician-led team.

Job Responsibilities

Deliver high-quality patient care in Skilled Nursing, Assisted Living, and Rehab facilities.

Evaluate and manage new admissions, follow-ups, and acute visits in collaboration with the attending physician.

Review hospital discharge summaries and align care plans accordingly.

Serve as the first-line provider representing PSG at assigned facilities.

Communicate regularly with patients, families, and interdisciplinary teams.

Coordinate care with primary care and specialists to ensure continuity and compliance.

Complete daily documentation and E/M billing within EMR according to PSG standards.

Collaborate with social workers and therapy departments for discharge and rehab planning.

Participate in quality improvement initiatives to enhance patient outcomes.

Qualifications

Certified Nurse Practitioner (ANCC or AANP)

Active South Carolina License (preferred)

Strong background in hospital, rehab, or acute care settings preferred

Excellent communication and interpersonal skills

EMR proficiency and strong documentation habits

Independent yet team-oriented work ethic

Passion for patient-centered care and improving post-acute outcomes

Working Hours

3 days per week (Monday–Friday, 8 AM–4 PM)
