Job description
PPE is seeking a dedicated and compassionate Nurse Practitioner (NP) to join our interdisciplinary team at Beacon Sub-Acute Facility. In this full-time role, you will provide high-quality medical care to residents in a supportive and collaborative environment. The NP will play a key role in improving patient outcomes through timely assessment, diagnosis, and treatment within our sub-acute care setting.
Job Responsibilities
Diagnose and manage acute and chronic conditions in collaboration with the attending physician
Prescribe medications and treatments in accordance with state and federal regulations
Monitor patient progress and update care plans accordingly
Communicate effectively with patients, families, and the healthcare team
Participate in interdisciplinary care meetings and contribute to discharge planning
Maintain accurate and timely documentation in electronic medical records
Ensure compliance with facility policies, procedures, and quality standards
Serve as a clinical resource and advocate for patient-centered care
Qualifications
Certification from an accredited NP program (e.g., ANCC, AANP)
Minimum of 1-2 years of experience in sub-acute, long-term care, or similar setting preferred
Excellent clinical judgment, communication, and organizational skills
Ability to work independently and collaboratively in a fast-paced environment
Knowledge of applicable regulatory standards and EMR systems
Working Hours
Regular Monday–Friday schedule — no weekends or overnights
Opportunities for continuing education and professional growth
You will see approximately 18-20 patients per day
