We are currently seeking a dedicated and compassionate Nurse Practitioner to join our team. This is an excellent opportunity for an experienced provider who is passionate about delivering high-quality care to patients in skilled nursing facilities.
Job Responsibilities
Perform physical examinations and diagnostic tests
Prescribe medications and treatments as needed
Educate patients on preventative care and health maintenance
Provide counseling and education for chronic conditions
Coordinate with interdisciplinary healthcare teams to ensure continuity of care
Deliver care on-site at skilled nursing facilities
Qualifications
Board certification (ANCC or AANP preferred)
Experience in geriatrics, primary care, internal medicine, or hospital medicine
Strong interpersonal and communication skills
Ability to work independently in a skilled nursing facility setting
Working Hours
8-4pm
No Nights / Weekends
