Overview:
Are you a dedicated advanced practice clinician seeking better work-life balance and more control over your schedule? We are currently seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join a well-established post-acute care team in Los Alamos, NM. This role offers schedule flexibility, professional autonomy, and the opportunity to provide meaningful care to a complex patient population.

Position Details:

  • Location: Los Alamos, NM

  • Setting: Post-acute care, inpatient (typically skilled nursing and rehab facilities)

  • Schedule:

    • Preferred full-time: 3.5–5 days/week

    • Part-time options available: 1–3 days/week

  • On-call: Daytime call only, Monday through Friday on assigned days

Responsibilities:

  • Provide comprehensive primary care in a post-acute setting

  • Manage both acute and chronic medical conditions

  • Create individualized care plans

  • Collaborate with an interdisciplinary care team (IDT)

  • Partner with supervising physician and facility staff

Qualifications:

  • Licensed NP (FNP, AGNP, CNP, APRN) or PA

  • Active New Mexico license or eligibility

  • DEA license preferred (not required)

  • Experience in post-acute care, outpatient, urgent care, primary care, home care, or inpatient settings is welcomed

  • Open to both experienced clinicians and new graduates
