We are seeking a full-time Nurse Practitioner to join our expanding team. This role focuses on providing compassionate, comprehensive care to patients in skilled nursing facilities, blending autonomy with collaboration in a supportive environment. The ideal candidate will have experience in primary care, internal medicine, geriatrics, or hospital medicine and a strong commitment to patient advocacy and continuity of care.
Job Responsibilities
Perform physical examinations and diagnostic tests.
Prescribe medications and treatments as needed.
Educate patients on preventative care and health maintenance.
Provide patient counseling and education on chronic illnesses.
Coordinate with other healthcare providers to ensure continuity of care.
Provide care for patients in skilled nursing facilities.
Qualifications
Current or eligible DEA license
Experience in skilled nursing, long-term care, geriatrics, internal medicine, or primary care preferred
Familiarity with EMR systems and documentation standards
Strong clinical judgment and diagnostic skills
Excellent interpersonal, communication, and time management abilities
Compassionate and patient-centered approach to care
Ability to work independently while collaborating with physicians and facility staff
Working Hours
8am-4pm
No Nights/ Weekends

Skills
Strong diagnostic and treatment planning abilities
Proficient in EMR documentation and E/M coding
Medication prescribing and chronic disease management
Effective communication with patients, families, and interdisciplinary teams
Health promotion and patient education
Time management and organizational efficiency
Adaptability in skilled nursing and long-term care environments
Team-oriented with leadership potential
Knowledge of Medicare/Medicaid guidelines and post-acute care standards
