Role Overview
- Location: San Franciso, CA
- Flexible full-time 4-5 day/week schedule Monday through Friday
- 15-18 patients per day
- Post-Acute care inpatient setting
- No call
What You'll Do
- Deliver comprehensive primary care
- Manage acute and chronic conditions
- Develop personalized care plans
- Integrate and work closely with the interdisciplinary team
- Partner and work collaboratively with a TeamHealth physician
What We Are Looking For
- Experienced post-acute care clinicians
- FNP, CNP, AGNP, APRN, or PA
- Active California NP/PA license
- Active DEA
