Join our dynamic healthcare team as a Primary Care Nurse Practitioner (NP) and play a pivotal role in providing high-quality, patient-centered care to individuals across the lifespan. As a Primary Care NP, you will work collaboratively with physicians, nurses, and other healthcare professionals to deliver comprehensive primary care services, promote health education, and empower patients to achieve optimal wellness.

Key Responsibilities:

  1. Conduct comprehensive assessments, including health histories, physical examinations, and diagnostic tests, to evaluate patients' health status and develop individualized care plans.
  2. Diagnose and treat common acute and chronic conditions, such as hypertension, diabetes, respiratory infections, and minor injuries, in accordance with evidence-based practice guidelines and regulatory standards.
  3. Provide preventive care services, including immunizations, screenings, and health maintenance counseling, to promote wellness and disease prevention.
  4. Manage chronic illnesses through medication management, lifestyle modifications, and patient education, ensuring continuity of care and adherence to treatment plans.
  5. Collaborate with interdisciplinary healthcare team members, including physicians, nurses, pharmacists, and social workers, to coordinate patient care, address complex health issues, and facilitate referrals to specialty services as needed.
  6. Utilize electronic health record (EHR) systems to document patient encounters, maintain accurate medical records, and track treatment outcomes, ensuring compliance with HIPAA regulations and privacy standards.
  7. Educate patients and families about health promotion, disease prevention, self-management strategies, and treatment options, empowering them to actively participate in their healthcare decision-making.
  8. Participate in quality improvement initiatives, clinical research projects, and interdisciplinary case conferences to enhance patient care delivery, promote best practices, and contribute to ongoing professional development.

Qualifications:


  • Current certification as a Nurse Practitioner (NP) by the American Nurses Credentialing Center (ANCC) or other recognized certifying body.
  • 6 months of general dialysis experience
  • Current licensure as a Registered Nurse (RN) and Advanced Practice Registered Nurse (APRN) in the state of practice.
  • Minimum of 1 years= of clinical experience as a Nurse Practitioner, preferably in primary care or family practice settings.
  • Proficiency in conducting comprehensive physical assessments, diagnosing acute and chronic conditions, and prescribing evidence-based treatments.
  • Strong interpersonal skills and the ability to communicate effectively with patients, families, and interdisciplinary healthcare team members.
