OPEN POSITION:
Nurse Practitioner - Geriatric Primary Care (SNF)
SCHEDULE:
- Part-Time
- 3 Days per Week
- No Nights or Weekends
- Some Flexibility Offered
COMPENSATION:
- $48 to $60 per Hour, dependent on experience
- Performance-Based Bonus Plan
- Malpractice Insurance

They bring quality medical care to the geriatric population, as well as people with complex conditions, special needs, and with intellectual and/or developmental disabilities. Their providers make a dramatic impact in increasing quality of life and safeguarding the dignity of those they serve. They are currently hiring due to growth in their Columbia market, where they’re in need of a provider to serve a skilled nursing facility.
POSITION DESCRIPTION:
The new Nurse Practitioner will see patients in one (1) skilled nursing facility, doing rounds and attending to acute needs on a part-time basis, working three (3) days per week during regular hours.
The practice believes strongly in proactive medicine, with a clinical focus on:
- preventative care
- chronic disease management
- medication management
- routine geriatric primary care
- quick responses to acute needs and changes
Providers see patients as often as is medically necessary, being assigned to specific buildings that they visit regularly.
The practice uses an electronic health record (a mobile-based platform specialized to this setting).
REQUIREMENTS:
- Kentucky NP license
- DEA
- 6+ months of advanced practice experience
- previous experience in SNFs, geriatrics, or a relevant area strongly preferred
