The Practice: Geriatric Medicine Associates (GMA) is an Internal Medicine clinic with two locations in Lakewood and Westminster, Colorado. There are a total of 12 physicians trained in both internal medicine and geriatrics and three nurse practitioners. GMA is currently recruiting for a nurse practitioner within the Westminster hospital-based (St. Anthony North) site.

Our practice is a member of the independent practice association (IPA), PHPprime. PHPprime provides wrap-around services to help make our practice successful, including care coordination, practice transformation coaching, risk adjustment coding education, population health management, data and analytics, specialist network management, and large group contracting.

The Position:
  • 1.0FTE opportunity – 32 patient facing hours/week
  • 8-10 patients/day
  • 4 day work week
  • EMR - Epic
  • Dedicated MA with care managers, social workers and phlebotomists on site
Qualifications:
  • Master of Science degree in Nursing from an accredited program
  • ANCC or AANP Certification
  • Must meet employer credentialing standards including hospital credentialing
