Hospital Medicine Nurse Practitioner / Physician Assistant

Looking for a rewarding inpatient role with a predictable schedule, strong physician support, and no overnight call?
InCare is seeking a full-time Nurse Practitioner or Physician Assistant to join our Hospital Medicine team at Encompass Rehabilitation Hospital in Concord, New Hampshire. This position offers the opportunity to practice in a collaborative inpatient rehabilitation setting, caring for medically complex patients while working alongside an experienced team of physicians, APPs, nurses, therapists, and case managers.

Job Responsibilities
Perform daily patient evaluations and follow-up visits
Manage acute and chronic medical conditions in the inpatient rehabilitation setting
Collaborate with physicians, nursing staff, therapists, and case managers to develop and implement treatment plans
Participate in interdisciplinary team meetings
Document patient encounters accurately and timely in the electronic medical record
Promote high-quality, patient-centered care and positive patient outcomes
Qualifications
DEA license (or ability to obtain)
Experience in hospital medicine, internal medicine, rehabilitation medicine, critical care, or a related specialty preferred
Strong clinical assessment and communication skills
Ability to work independently while collaborating within a multidisciplinary team
Commitment to compassionate, high-quality patient care

Familiarity with post-acute and rehabilitation patient populations
Experience coordinating care with therapists, nursing staff, case managers, and specialists
Understanding of quality metrics, patient safety, and discharge planning
