Position Summary
The APP Hospital and Managed Care Liaison will play a central role in managing inpatients at Advent Health Ocala. While the scope includes a broad responsibility for the inpatient population, there will be a particular emphasis on coordinating care for managed care patients. This includes proactive communication with managed care companies and their case managers to optimize patient management and ensure a seamless continuum of care.
Key Responsibilities
Inpatient and Managed Care Coordination:
- Manage the care of inpatients, with a specific focus on those under managed care plans.
- Identify managed care patients upon ED presentation and coordinate with their primary care providers (PCPs) to explore outpatient care options whenever clinically appropriate.
- Serve as a liaison between the hospital, managed care teams, and PCPs to ensure coordinated and efficient patient care transitions.
Inpatient Management:
- Admit, assess, and manage inpatients, including daily rounds and discharge planning.
- Coordinate with a multidisciplinary team to ensure comprehensive care, appropriate resource utilization, and adherence to quality standards.
- Ensure accurate documentation and communication across all care touchpoints.
Collaboration and Support:
- Engage regularly with managed care companies and their case managers to align inpatient care with managed care protocols and value-based care models.
- Provide support to the hospitalist team as needed, assisting with admissions, rounding, and cross-coverage for non-managed care patients based on patient volume and acuity.
Quality and Compliance:
- Adhere to hospital policies, regulatory requirements, and participate in quality improvement initiatives.
- Ensure compliance with managed care documentation standards and value-based care goals.
Qualifications:
- Current licensure as a Physician Assistant (PA) or Nurse Practitioner (NP) in Florida.
- Relevant certification and a minimum of 2 years of experience in hospital or internal medicine, with familiarity in managed care strongly preferred.
- Strong clinical, communication, and organizational skills.
Work Schedule:
- Monday to Friday, 11 am – 7 pm
