About the Role
This role is responsible for translating clinical strategy into scalable, repeatable, and enforceable operating systems that drive provider productivity, optimize patient retention within preferred partner networks, and ensure consistent care delivery across inpatient, outpatient, home-based, and post-acute settings.
The Director of Clinical Operations owns day-to-day operational performance, including workflow design, provider utilization, retention tracking, and cross-setting care coordination. This position serves as the primary execution partner to the Chief Nursing Officer (CNO), ensuring operational discipline, identifying gaps, and implementing effective solutions as the organization scales.

Key Responsibilities
- Develop and maintain clinical and operational policies to ensure consistency in care delivery, documentation, billing workflows, and regulatory compliance
- Design, implement, and manage workflows, referral pathways, triage processes, and scheduling models across all care settings
- Drive provider productivity, utilization, and patient retention initiatives
Qualifications
- Licensed Nurse Practitioner (APN) in New Jersey (required)
- 3–5+ years of experience in palliative care, hospice, geriatrics, or complex care settings
- ACHPN (Advanced Certified Hospice and Palliative Nurse) preferred; required within one year of hire
- Proven experience managing multi-site clinical operations
- Demonstrated ability to build and enforce scalable workflows
- Strong track record in provider productivity, utilization, and patient retention
- Excellent organizational, communication, and problem-solving skills
- Experience across hospital, skilled nursing facility (SNF), and home-based care settings preferred
- Proficiency with EMR systems such as Epic Systems, PointClickCare, and Homecare Homebase preferred
